Most of us don't plan for aging until a crisis forces our hand — and by then, the emotional, legal, and logistical gaps are already costing families dearly. This presentation explores what it truly means to be prepared: building the confidence, structures, and support systems that carry us from intentional planning all the way through end-of-life with dignity. Sun-Hee Yoon and Tiffany Johnson will walk the room through the full continuum — from proactive care planning to the powerful role of death doula work and social hospice — showing how women in business can lead this conversation for themselves, their families, and their clients.

Meet our Presenters: Sun-Hee Yoon and Tiffany Johnson 


 

Sun-Hee Yoon is the Founder and CEO of Sage Lifestyle Concierge LLC, a comprehensive care planning and lifestyle navigation service helping individuals, families, and professional advisors move through the full continuum of aging with clarity and confidence. A Dementia Connection Specialist, she is the creator of Charting Your Courses at the Union League Club of Chicago and the host and producer of the Sage Pathways Podcast. She serves on the boards of the Oak Park River Forest Chamber of Commerce, Arbor West Neighbors, and the COC West Cook Chapter, and is a member of the Illinois Family Caregiver Coalition's Advocacy Task Force and the Community Engagement Council at Rush Oak Park Hospital.

Sun-Hee's work is rooted in lived experience. When her husband needed care, she discovered firsthand how fragmented and overwhelming the journey could be — not just logistically, but emotionally, legally, and socially. That experience became the foundation for Sage's integrated approach, which addresses legal and financial planning, healthcare navigation, caregiver support, and lifestyle design as a single, connected circle of support. She brings both professional authority and personal authenticity to every room she walks into.



Tiffany Noelle Johnson is the Cofounder and Executive Director of Wally’s Hearth, a 501(c)(3) nonprofit opening Illinois’ first social model hospice home in the Oak Park area. Wally’s Hearth will provide a loving, high-quality, home-like place for terminally ill guests in the final days and weeks of life, offering nonmedical care, comfort, meals, presence, and family support while hospice agencies provide medical care. The work is rooted in dignity, inclusion, and deep respect for dementia, cultural and faith traditions, chosen family, LGBTQ+ communities, and the tender complexities families bring to the end of life. Through We Too Shall Pass, Tiffany also serves as a death doula, deathcare educator, funeral celebrant, and willow casket weaver, helping families prepare, gather, grieve, and make meaning.
